6 week post partum check-up

Monday, June 3, 2013

I can't believe that I already had my 6 week OB appointment today.  B will be 7 weeks on Thursday!  Where has the time gone?

I was really nervous for my appointment this morning for a few reasons.  The first reason was because I had to go alone with B.  I have gone alone with him to appointments before, but never to the hospital and never for 4 1/2 hours.  I had to go early this morning to first get my 2 hour GTT and then for my OB appointment afterwards at 11:20 AM.  So yeah, I had to entertain B for several hours, in a hospital setting, by myself.

Turns out it wasn't as bad as I thought it would be.  I nursed him a total of 2 times and changed 3 diapers in the family restrooms.  Luckily everything in the hospital is wheelchair accessible!  I hate the nursing covers and would prefer to just "free-boob" it, but I am not sure how legal that is in my state (or anywhere for that matter).

The 2 hour GTT test wasn't as bad as the 3 hour, but the drink was by far worse.  It wasn't the orange flavored one, it was no flavor other than sugar water.  I choked it down and it threatened to come back up for several minutes.  Luckily I could drink water to take the sticky, sweet taste out of my mouth.  GROSS!

After chugging the gross drink, B and I did several laps around the hospital.  We walked past the fertility clinic.  I wanted to go in and thank my RE and their staff, but didn't think they would even remember me since it was September that I was there last.  I saw the doctor that delivered B and I also saw a few other doctors that I remembered walking the halls.  It was weird to be back there again for what will be the last time for quite a while (or until I try to get pregnant again or get pregnant again on my own).  The site of so many heartbreaks and bad memories, now has a bunch of great memories for me.  They helped me conceive and birth my son.  I am forever grateful.

After my last blood draw, I headed over for one last visit to MFM.  I am not sure what my BP was, but my weight is 133 (down from a high of 164 plus).  I still have 13 lbs to lose to get back to my pre-pregnancy weight, or 15 lbs to lose to get back to my goal weight.  I realize that this weight should not come off until I stop breastfeeding and I am ok with that.

I talked to my doctor one last time.  We discussed birth control, as he gave me the all-clear for sex (which I still have no interest in right now...sorry hubby!).  I am going to start the mini-pill on Sunday.  Not really excited about that, but I also do not want to get pregnant for at least a year and having something put into me like an IUD sounds completely unappealing.  We also discussed exercise and my diastasis recti.  I told him about the marathon and he told me that I can start running...slowly.  I am surely going to be at the pace of a turtle for some time.  As for the ab situation, he told me that he has never seen anyone have luck with physical therapy, but he agreed to write me a prescription if I agreed to call him and let him know if it worked or not.  He also told me that if I choose to have another baby and a c-section that he will stitch my abs back together as part of the surgery, if that was going to be my last child.  I love this man!!  He also checked my incision, which he said looked good and he gave me a pap smear which hurt like hell.  He told me to give them a call if I get pregnant and we'll do it all over again starting at 8 weeks.

I really felt like hugging him, but I thought that would be weird.  Seems like so long ago that I walked in there for the first time, crying and so scared.  He really always helped to calm me down.  He never tried to blow smoke and sugar coat anything, but he also didn't unnecessarily scare me either.  It is crazy that you spend so much time at their office when you are pregnant and then you just stop going.  It sounds weird to say that I will actually miss going for regular check-ups there.

When I handed the receptionist my paperwork and told her that I did not need to schedule a follow-up it felt like I was graduating.  I now have my healthy baby that I worked so hard for.  Quite a surreal feeling.  I did it.  I overcame whatever issue I had.  I fought and I won.

When I got home I already had an email from the nurse giving me the results of the 2 hour GTT.  I PASSED!  NO MORE DIABETES!!  My fasting was 87 and my 2 hour was 112.  I breathed a sigh of relief, but know that I will forever have changed my eating habits and will always be thinking about diabetes.  I know that I am at a higher risk now for type-2 diabetes, so I have even more incentive to stay healthy.

Today was a good day.  B has been doing better the past few days.  I have a more thorough update of him that I need to write, but I hear him stirring from his nap so I need to get back to mommy duty.

For fun, here are some before and after shots of my mangled belly.  Something to work my ass off for.    I am jealous of my old body!  Enjoy!

What a difference a year can make

Tuesday, March 19, 2013

Yesterday I had my 36 week appointment (36 weeks, 3 days).  From here on out I have weekly biophysical profiles instead of NSTs and bi-weekly growth ultrasounds due to the GD.  I like having them on Monday mornings because it gives me something to look forward to regarding the beginning of the week!

The BPP this week was done by a guy.  I don't think that I have had a guy do an ultrasound for me since during my miscarriage days.  I was sort of disappointed because my experience with the guy wasn't great, but also the circumstances were not great that time either so that probably jaded me.  This guy was really friendly right off the bat.  He asked me the usual questions and after my usual pre-ultrasound bathroom break, he went to work, checking out baby boy.

He passed the BPP with flying colors.  He was practicing breathing and we caught him yawning several times.  He was sucking his thumb most of the time.  Head is still down.  The only thing that was slightly concerning was the measured amniotic fluid level is now on the high side of normal up from the low side of normal.

Without me asking, the tech flipped to 3D and was determined to get a good shot of this little guy's face.  After a little coaxing the baby's hand out of the way, he was able to get a great shot (scroll to the bottom).  My baby looks like a real baby and not a ghost!

I met briefly with the doctor who was happy with the BPP results.  We briefly talked about the amniotic fluid and he said that he was not concerned since it was still in the normal range and it is a subjective measurement that differs from tech to tech.  He said that if they saw an abnormal amount that they would have mentioned it on the report for sure (they are a high risk u/s and see thousands of high risk patients a month).  We talked about my blood sugar readings which have been getting better.  My fastings have been in the 70s/80s for the past week.  He said that this could signal that the placenta is producing less hormones and that labor/delivery might not be that far away.  He said to keep monitoring it and they will check next week.  Based on my blood sugar readings he guessed that I might not make my due date.  I told him I would not be upset about that.  :)

My next appointment is next Monday...37 weeks, 3 days.  I'll be full term!!  It is amazing what can happen in a year.  Last year at this time I was one day away from finding out the baby that I was carrying was not viable.  The heartbeat was only 80 and was small for gestational age.  That time was one of the darkest in my life and I have never been so depressed.  I never thought that a year from that point, I would be carrying this little guy and be almost ready to meet him.  I am so incredibly blessed to be where I am right now.  As hard as it was to get here and how this pregnancy has not been the easiest at times, I know that I am so incredibly lucky.  I honestly thought that I would never be able to carry a baby past 10 weeks.  I thought I was defective.  I don't know what happened and I am not sure that I will ever know.  I do know that I have a beautiful baby boy on the way and I am so thrilled to hold him, kiss him, and be his mommy.

Yet another visit to MFM

Thursday, February 28, 2013

Yesterday's visit to the doctor was not what I expected.  I thought that I was getting another growth ultrasound, when in fact I was not.  I just had a regular ultrasound where they checked the welfare of the baby (including the heartbeat and the fluid level).  The baby checked out great.  We got to take another look at his cute (but large) head.  He had his hands up in his face which he likes to do when they try to disturb him with the ultrasound wand.  I am wondering if he is a thumb-sucker.  I was!

I asked the radiologist if she could check on my cervix while she had the wand out.  I explained to her my cramping ordeals.  She said she would try, but that at this stage in the game, it is hard to get a good look at the cervix, not to mention super uncomfortable.  She tried, but with no real luck.  She mentioned that the doctor would do an internal check which would give me a better idea of what I was working with.  Oh joy, my first internal!

On the way to the doctor appointment, I was having more cramping.  As my husband drove over the many craters and bumps on the way from my work to the hospital, all I could think about was how uncomfortable it was now with mild cramping and how incredibly miserable it would be while in full out labor.  Something to look forward to in the coming weeks.

The doctor appointment went well.  I gained 1 lb in a week (gulp).  Hopefully it was me gaining the weight and not the baby (maybe the cookies I made and have been enjoying every night :).  My blood pressure was good.  The doctor came in and we first discussed how the insulin was going and how my numbers were looking.  He was quite impressed with my 1 hour post-meal readings. Looking at most of them, would suggest that I am not even borderline GD.  This is due to the strict diet, though.  My fastings on the other hand are a whole different beast.  Still very difficult to control, even with the insulin.  Yesterday's reading was 93.

We discussed the times that I was sleeping and I explained to him my wacky schedule.  When I go into the office, I go to sleep around 9:30 PM and get up at 4:30 AM.  When I work from home, I go to sleep at 10:30 PM and get up at 6:30 AM.  On the weekends, I go to sleep around 11 PM and get up at 7 AM.  He asked if there was any way that I could go to sleep around 10 PM every night, taking the insulin at that time, and get up at 6 AM.  I told him, yeah sure if you write me a note so that I can work from home for the remainder of my pregnancy!  He said he would do that, but he first wanted to try me taking the insulin earlier, at like 8-8:30 PM if I am getting up at 4:30 AM.  I agreed to try this first.  I was hoping for the early WFH pass.  If I am still having trouble with things then I may take him up on the offer.  Today my reading was 88, with taking the insulin at 8 PM.

Next we talked about the cramping and contractions.  He asked some of the same questions the nurse asked and said he thought it sounded normal, but that he wanted to do an internal check to be sure and put my mind at ease.  I was expecting a ton of pain with this, but it didn't hurt more than a pap smear.  Turns out I am not dilated, AT ALL.  My cervix is tightly closed and still very long.  His head is way down low, though, which may be adding to my discomfort.  I am not sure how I feel about this.  I figured I would maybe be a centimeter (knowing that you can be a centimeter for weeks), so now I am back to thinking induction again.  Crap.  I am actually surprised that after 2 D&Cs and being forcefully dilated both times, that things are still so...umm... tight in there.  I guess my surgeons both knew what they were doing.

Knowing the state of the baby, my blood sugar, and my cervix, I don't have another appointment until 3/11.  I will be getting yet another ultrasound at this appointment and I assume that they will check the growth this time.  I think I am going to dial back my work-out routine until he is born.  I am not sure what keeping the 5 miles of walking a day is a good thing at this point.  I am going to cut it down to 3ish and see what happens.  I still need to get moving or I feel anxious!

No news is good news?

Wednesday, February 6, 2013

Yesterday I had my regularly scheduled visit to MFM @ 30 weeks, 4 days.  No regular ultrasound, just got to see this little guy with the mini.  My blood pressure was better than usual at that place, 126/68 with the stupid automatic blood pressure taker that is always much higher than the manual one.  I can literally feel my blood pressure rising when I step into that building.  I tried a little meditation in the waiting room, but they had H.ouse H.unters on so it was tough for me to concentrate.  I am surprised it was so good.

I had lost about half a pound since my last visit a month ago, right now I am still up about 20 lbs. total.  The doctor wasn't worried.  He said that it was the GD diet causing me to lose weight.  He said the baby is still gaining what he needs as long as I am eating healthy.  I am totally fine with that.  Less weight to lose after the baby gets here.  Thinking back, I was really afraid of gaining too much weight and now I am not worried about that at all.

The we took a peak at the baby with the hand-held ultrasound.  His head is still down (yay!).  His butt is lodged right against the front of my stomach.  Now I know why it is extra hard in that spot.  His heart was beating nicely and he had a good amount of amniotic fluid.  He doesn't like the ultrasound so he promptly kicked as soon as the wand went down.  He hates the Doppler, too.  I heard that if it gets close to their ear it sounds like a passing train.  Poor little guy, I don't blame him for kicking it out of the way.

After checking on the baby, the doctor asked to look at my blood sugar sheets that I have been filling out.  He wasn't concerned.  He said to keep tracking and that if I notice any recurring spikes to give them a call because I would most likely need insulin.  He also said that if I notice my levels going lower and lower to give them a call because that could mean impending labor and placental degradation.  Yikes, so much to worry about.  I guess I just hope to stay right where I am.  Here is a peak at my blood sugar levels over the past few days for those that are interested (I know I am always interested to see them from others now).  Keep in mind that I am having almost no carbs, so this is why they look so good.  If I so much as sniff a carb, I go above the limits.  Besides the fasting, the other numbers are 1 hour after meals.

Before I left, the doctor and I discussed a few things, including me getting the pertussis booster vaccination.  I am usually not really keen on vaccinations, but I have read a few things that might make me change my mind.  I may end up getting this at my next visit.  Vaccines during pregnancy still scare me.  The doctor also suggested that my husband get the flu vaccine.  Yeah right.  I don't think there is a chance in hell that he will do that since he is just anti-vaccine, in general.

I scheduled my next appointment and growth ultrasound for 2 weeks out.  I'll be almost 33 weeks at that point...total craziness.

When it rains it pours

Wednesday, January 9, 2013

The past week or so has not been one of the greatest on record for me, so I apologize in advance for this post because it is a rant with lots of whining.  It started with me coming down with shingles (or not...I'll get to that), then me coming down with a horrendous head cold that I can't seem to get rid of, to my last MFM appointment yesterday.

Last Thursday, I had an appointment with my PCP to diagnose the weird rash on my neck that I had since NYE.  She was fairly certain that is was shingles so she put me on A.cyclovir (5 times a day for 10 days) and also on an antibacterial ointment, M.upirocin (3 times a day), in case I had impetigo or another related infection.  She also did a viral and bacterial culture on it to be sure of the diagnosis.

Yesterday morning I got a call from her office telling me that both of my cultures came back negative. The nurse said that the doctor was still fairly confident that it was shingles and I just didn't have a raging case and to continue taking the meds until they run out.  She also said I should continue the ointment, as well.  Lovely.

Now on to the head cold.  This started on Friday.  In the matter of a few minutes I went from feeling fine to feeling like I had to sneeze to being all stuffy.  I went out and bought B.reathe R.ight strips, a humidifier, and V.icks V.apo R.ub.  I know that they say you can take T.ylenol C.old during pregnancy, but I am already on so many other meds that I don't want to take anything else.  I have not slept more than an hour at a time since Thursday night.  I missed work Monday and today.  I feel like total crap.

Next I'll discuss the MFM appointment.  The ultrasound went great.  In fact, they used me and my little boy in a teaching demonstration.  His heart, brain, arms, legs, spine, kidneys all looked great.  He measured in the 63 percentile (~2.5 lbs) with a HB of 155.  Here is a pic of him (scroll to the very bottom).

After the ultrasound I went to the MFM office.  They checked my blood pressure and of course it was high as it usually is in their office, 133/66.  My weight was 139, which was better than I thought.  So I am up 19 lbs.  Then they gave me the orange drink for the 1 hour GTT and I chugged it in under 2 minutes.  Totally gross and more sugar that I have had in quite some time.

I saw the doctor and he was happy with the ultrasound and everything else.  He checked my rash and concurred that it was most likely shingles.  He explained that if it is a low level infection that your body is already fighting successfully that the culture could very easily come back negative.  He told me to continue the meds.  He said he didn't need to see me for another month and that was that.  I then peed in a cup (as per usual) and once the hour was up I got my blood drawn.

This morning I had a message telling me that I (quite miserably) failed my 1 hour GTT and to call and schedule a 3 hour test.

GTT Screen (GLT): 163 (Normal Range = 54 to 139): OUT OF RANGE

After another night of no sleep and another day of calling in sick to work this was just too much for me to handle.  I called my mom and sobbed to her.  Half the time she couldn't tell what I was saying.  I was embarrassed and upset.  I eat so healthy 99% of the time.  I don't drink soda, ever.  I NEVER eat fast food.  I eat so little processed food and what I do eat that is some what processed is usually organic.  I eat a very high fiber cereal at breakfast.  I eat lots of fruits and veggies and I was of a healthy weight when I got pregnant.  Up until I got sick I have been exercising 4-5 times a week with 2-3 mile walks and some light weight-lifting.

I seriously feel like a failure.  What did I do wrong?  I called the nurse to set up my 3 hour test and they forced me to schedule an appointment with the nutritionist as well.  She told me to stay away from food products where the first ingredient ends is "ose" and simple carbs and to "start eating healthier".  I started crying on the phone with her because I already do that.  My friends make fun of me because I eat so healthy.

I told her that I already eat healthy and exercise and that I haven't gained an obscene amount of weight.  I think she felt bad for me because she could hear how congested and nasally I was.  She told me that she sees 300 lb women pass the test with flying colors and 90 lb vegetarians fail miserably.  She said it doesn't have to do with weight, but how your body handles sugar during pregnancy.  Yeah, yeah, yeah.  That didn't really make me feel better.

So now I have to further restrict what I eat.  No more cereal or wheat crackers for me.  Back to the lovely world of constipation.

I have spent my day feeling down and miserable.  This cold is kicking my ass and now I have to worry about diabetes, too.  How is it that some women get pregnant by sneezing, share their wet pee-stick with their 5,000 FB friends, buy nursery furniture before 12 weeks, eat McD.onalds every day, don't exercise, and have completely healthy babies (over and over and over again).  Then others have a hard time getting pregnant and have problem after problem during their pregnancy?  It isn't fair.

Sorry for the vent and kudos to you if you read this whole thing.  I am not feeling great today.  Hopefully tomorrow will be better.

MFM visit @ 22 weeks, 4 days

Thursday, December 13, 2012

This past Tuesday I had my regularly scheduled MFM visit with my doctor.  No ultrasound this time, so I was in and out fairly fast.  My blood pressure was still fairly consistent with where it has been at every visit to that place, higher than normal.  Tuesday it was a little lower than it has been at their office at 136/66.  They tend to use the machine when I go there and then they sometimes follow up with the manual one.  The machine always has a much higher reading than the manual one.  I am not sure why that is.

The next order of business was checking my weight.  I made sure to shed every non-essential piece of clothing for this, which is something that I have never done before.  For the first time in my life, I weigh more than 130 lbs (134 lbs to be exact).  I know, for some that doesn't sound like a very high number, but for me, the life-long athlete, it not the easiest thing to see.  I asked the nursing assistant how that number measured and she laughed (hopefully not at me) and said it was somewhere in the middle.

After all of that fun ended, the doctor came right in, which is not normal.  He is normally the doctor on call in the hospital.  Usually I wait around for a while, but I guess that today was a slow day or something.  He came in with a medical student, which I agreed was fine.  He mentioned my thyroid and I let him know that I was seeing an Endocrinologist who was seeing me every 3 months, still had me on the 25 mcg. of Levothroxine, and at my last check at the end of October my TSH was 1.07.  We was happy with that and I impressed the medical student by knowing my exact numbers.

The doctor had me lay down and he checked on the baby with the hand-held ultrasound.  Wow was this guy active while we were there (and then for the rest of the day).  He punched and kicked all over.  We even saw my stomach moving from the outside again.  The baby's head was down and off to the right and his feet were up to the top left, which was where we saw most of the outside movement.  I mentioned the Doppler incident and he mentioned that he doesn't recommend those to his patients for the very reason that I got worried the other night.  He checked the heart and said that it looked healthy to him, be he also said that during this time it can get irregular occasionally since it is working so hard to help baby grow.  At this point in time, however, it was beating away normally.  Phew.

We talked briefly about my weight gain, at my request.  I asked him what he thought about it.  I am up about 14 lbs.  He said he would like me to gain 25 lbs.  I gulped.  Note sure how I managed to gain 14 lbs already after losing weight in the 1st trimester.  I am also not sure how I am going to be able to only gain 11 lbs more during the rest of this pregnancy.  Any one have any tips or advice?  I walk at least 4 days a week for at least 2-3 miles at a time.  Plus, I walk to and from the train on most days which is a little under 2 miles round trip.  I eat healthfully (for the most part).  No soda and not a lot of sweets.  My typical day consists of (M-F):

5:30 AM
high fiber cereal (a God send)
fruit (usually a banana, berries, or an apple)
orange juice w/ calcium & vitamin D
water

8:30 AM
S.iggi's yogurt
berries
water

11:30 AM
leftovers from dinner or a salad
water

1:30 PM
fruit
Kind Bar
water

5:00 PM
hand full of trail mix
or
cheese and crackers
water

7:30 PM
chicken or fish (usually)
veggies
rice or quinoa
water

**Occasionally, I have a sweet treat or two in there like a snack sized peppermint patty or half of a Whole Foods triple chocolate brownie (YUM).  Also thrown in there as I prepare dinner might be some olives or some Cape Cod Reduced Fat potato chips.**

After that he asked me if I had any other concerns.  I mentioned my Vitamin D levels might be low and he offered to check them for me (it ended up being  on the lower end of normal at 38 ng/mL, normal being 20-80 ng/mL).  He told me to come in at the sign of any bleeding, prolonged pain, regular contractions, if I fall (in any direction), or for any other reason that I might feel concerned (gosh I love that place!).  He told me that he'd see me in a month and that we would do an ultrasound before the visit so he could better check how things were progressing.  I go back on 1/8.  I am excited to see my growing little boy!

My first MFM visit

Wednesday, September 19, 2012

Yesterday was my first trip to the Maternal Fetal Medicine office.  I wasn't scheduled for an ultrasound so I went straight to the office to fill out mounds of paperwork.  I was early to arrive so I was very surprised when they called my name before I was done signing all the documents.

They weighed me and I still weigh exactly the same (with my clothes on) as I weighed before I got pregnant.  They told me that this was normal and that I shouldn't really expect to gain anything until the second trimester.  The nurse's assistant took my blood pressure, but I don't think that she did it correctly.  The band wasn't very tight around my arm and she didn't ask me to roll up my sleeve.  Then when she inflated it (while I was still trying to get situated on the table and she let some of the air out then re-inflated it 2 times.  She asked me if I had high blood pressure and I told her no, actually I usually have slightly lower blood pressure.  She kind of shrugged her shoulders.  I just had my blood pressure taken less than 2 weeks ago and it was normal so I am thinking it was operator error?  No one in my life has ever told me that I have high blood pressure!

Then the nurse came in to ask me a ton more questions, but suggested that I go pee in a cup first.  That is always a fun experience.  I won't get into too much detail, but suffice it to say, I have very bad aim.  The nurse had told me to leave the cup in the bathroom when I was done.  It felt really odd to just leave a cup of my pee on the shelf in there, but I did it, reluctantly.

Then came the rest of the questions from the nurse which ranged from drug and alcohol use to mental health to if we rented a house or owned.  The nurse was really nice and we had a bunch of things in common.  We were both from the same state originally (and in the same part of the state) and we both graduated from the same college.  It is a small world!  The nurse also asked me about my losses and when and how they occurred.  That is always a hard conversation to have especially since I am coming so close to the anniversary of my first D&C.

After the nurse went through all the questions she explained to me about the Downs Syndrome screening that I would have at 12 1/2 weeks, followed by another blood test at 15 weeks, and then a gender scan at 18 weeks.  I feel like I still tune out those types of events that are so far into the future because I still feel like I can't be certain that I will get there.  I think that is just my learned defense mechanism working.

After she explained some of the future appointments, she left me to wait for the doctor.  I figured I would be waiting for a while, but the doctor popped right in.  He was awesome.  I was at ease from the moment he came in.  Very no nonsense and extremely understanding of my situation.  The first thing he wanted to do was check on the baby with his incredibly awesome hand-held ultrasound machine that looked similar to the one in this picture.

So cool!
My heart skipped a beat when he put the wand down on my stomach, but he picked up the baby right away.  The baby was moving all over the place and its little heart was beating away.  So amazing to see that little baby again!  Immediately, I asked him if he thought the heart rate looked slow (always my biggest learned fear), and at first he looked at me funny and said it looked completely normal, then he realized why I asked and reassured me some more that it looked great.   He showed me a few different angles of the baby before he put the machine away.  I want one!

After that, we talked for a little while about my history and about this pregnancy going forward.  He debunked some myths and told me all soft cheeses were safe as long as they were pasteurized.  He emphasized me to eat pasteurized foods/drinks, if there was an option.  He also told me that anything that I feel like eating during the first trimester is fine.  He said there is no difference in babies where the mom's had trouble eating in the first trimester than those that didn't.  He actually said he feels better about pregnancies where the mothers are nauseous because it means a better connection between mom and baby.  That made me feel better.

We talked a little about my ongoing risk of miscarriage and he didn't think that it was different from any other normal pregnant person at this point.  We joked about statistics, but he mentioned a 96% chance at a take home baby at this point.  I know nothing is certain (EVER), but it made me feel a little more comfortable.

He told me to call their emergency number 24/7 if I ever felt like there was an issue and/or if I ever had bleeding of ANY kind.  They have a 24 hour prenatal care center at the hospital that is open to all the patients.  He told me to call them first if I was going out of town, but said he preferred his patients not travel.  I haven't told my husband this as we were planning on going on our postponed trip to Turks & Caicos in November.

After meeting with the doctor I scheduled my next scan and appointment for Oct. 2 and then headed over to get blood taken.  I have been tested for STDs and AIDs so many times in the last year because of all my other pregnancies, but I guess they are required to do it.  At least I have already tested NEG for Cystic Fibrosis, so I don't need that again.

Overall I thought the visit went really well.  I really like the doctor and the nurse.  I think they treated me well and were very understanding of my situation.  I really like that hand-held ultrasound gadget.  I know that even if I don't have a scan that they can check on the baby.  I am very happy with my decision to stay with the hospital, so far, and I am cautiously getting excited about meeting this little one in a few months.  :)

MFM vs. OB

Thanks to everyone who offered me advice in one of my previous posts regarding my choice of staying a the hospital and sticking with the maternal fetal medicine practice or going back to my regular OB.  I was worried that I might make the wrong choice, but after my conversations with both offices yesterday, I am pretty confident that I have made the right decision for me right now.

I called the MFM practice last Friday at lunch and left them a message with all of my information.  Their answering message said that someone would call you back within 24 hours.  I assumed those were business hours, so I waited until Monday at lunch to give them another call back and leave another message.  By the time that I was ready to leave for my acupuncture appointment yesterday, I had not heard back from MFM.  I did, however, receive a reminder phone all from my OB about my annual exam that I forgot was scheduled for Wednesday.

On my walk to acupuncture, I called the OB and told them about my situation.  Since I was so frustrated with not being able to get in touch with MFM, I asked if I could convert my annual appointment to an OB intake appointment.  Unfortunately, they would not let me do that.  The scheduler let me speak to a nurse to figure out what I should do next.  The nurse was nice, but not very helpful.  I told her about my three losses and about how I was being seen by a RE.  She asked if I had been through IUI/IVF and I told her about the Femara.  She asked about any drugs I was on currently and I mentioned the Levo.  She told me that I would be fine to start going back there for regular prenatal appointments.  I told her that my only caveat to going there was that I needed regular ultrasounds, at least until I was out of the first trimester.  She put the breaks on right there and told me that it would not be possible to get an ultrasound there until 18 weeks!  I started to get worked up and told her that my RE recommended one at 11 weeks.  She said that insurance would not cover another ultrasound until 18 weeks unless I was bleeding or over 35.  She said they did not treat infertility/early loss patients as high risk.

I was about ready to cry with frustration at this point.  There was no way I could wait until 18 weeks for my next ultrasound.  I was also totally out of breath from walking over a mile to acupuncture and struggling to hold back tears.  She told me I might best be taken care of at the hospital.  I told her I was trying to get an appointment with no luck.  She commiserated and suggested I call back in a day if I had not heard from them.  I agreed and tearfully hung up the phone.  I was so shocked that even after 3 losses in a row, they would not try to work with me to get another early ultrasound covered by insurance.

My luck would have it that the MFM practice decided to call me back right as I was walking into my acupuncture appointment!  I stepped out so that I could take the call.  The woman apologized for the late returned phone call.  She was well informed of my situation (probably due to my 2 very detailed messages...haha).  She happily placed me with the high risk doctor (who is also a full service OB) that my RE recommended.  I have an intake appointment for next Tuesday and then that will follow with an ultrasound after that.

Such relief!  I think that for me, I need to stay with the hospital.  I think they already get my situation and are willing to work with me to ensure that things are covered by insurance.  They have a state of the art NICU that can handle babies born at 23 weeks or beyond.  Hopefully I won't need that, but it is nice to know.  I am also closer to the Children's Hospital, if necessary. Yeah it might be a pain to have to come into work to go to an appointment, but I am already used to making the trip there from work anyways.  Now I just hope and pray that this baby continues to thrive!
